EICS Application for Verification of Credentials - Affidavit and Release
As part of the EICS Application for Verification of Credentials, applicants must provide an Affidavit and Release signed and certified by a 'public notary, first class magistrate or consular official'. We would be pleased to assist you to witness and certify your EICS Application for Verification of Credentials.
Our fee to witness the EICS Affidavit and Release is AUD60.00 inclusive of GST.

“The New South Wales Medical Board requires that medical practitioners seeking medical registration who completed their medical education outside Australia or New Zealand submit copies of certain documents to the Educational Commission for Foreign Medical Graduates (ECFMG) International Credentials Services (EICS). EICS will obtain primary source verification of the authenticity of these documents from the entity that issued these documents to you.”

Our notary public fees are LOWER than the recommended notary public fees.
We would be pleased to provide you with an estimate for any notarial service that you may require. Quotes will only be provided on physical assessment of your documents. All services are provided by appointment only. Appointments outside normal business hours, or by 'home/office visit' by a mobile notary may incur additional fees. We generally accept payment by cash, or by company cheque. Credit card payments incur a surcharge.
